Low- and mid-back pain usually comes from a small number of specific structures - a facet joint, a nerve root, the sacroiliac joint - and an injection treats the one at fault directly rather than medicating your whole system. Calming the inflammation often opens a window in which physical therapy finally works, which is where the durable improvement comes from. Every injection is image-guided, done in the office, and tells us more about the source of your pain.

Spinal cord stimulation is for back and leg pain that has not responded to injections, therapy, or surgery. A small implanted device interrupts pain signals in the spinal cord before they reach the brain. You are never committed on day one: a short trial with temporary leads lets you judge the relief during your normal week, and only a trial that genuinely helps leads to a permanent implant.

When conservative care has run its course, these procedures address the structural problem - a crowded spinal canal, an unstable segment, a painful vertebral endplate - through small incisions rather than open surgery. Less disruption to muscle and tissue generally means less post-operative pain and a faster return to normal activity. They are considered only after the less invasive options have been tried.
